Silent Old Sanctuary one weapon only + level up Empty Tome to max.

You have to end the run with the maximum Cooldown bonus; starting with SOS and picking up the Empty Tome should eventually get you there, but you can't start collecting weapons once you have it.

Ah, So it MUST be just that item. I get it now. Yeah. So assist items are fine like spinach just never touch other weapons, aim for tome?

Yeah.
Any passive/hidden weapons are fine.
Active (in upper slots) are not.
Do not forget you can check 1 weapon only in character select so you'll not get any active weapons in level up choices eventually.
