Go back and do the Death fight again with Richter. I did it with Inverse on so I could buy the extra Arcana at the beginning, and completed 292/292 bestiary after the credits rolled.

Finally, worked for me as well. :) 100% done, time to wait for next dlc.

I had fought with him once to unlock the lots of characters, then again to unlock Death as a character and it was still unmarked. I had to defeat him a third time and only then the bestiary entry got unlocked.

Maybe this hasn't been patched on PS4 yet (we have the Bitterfly typo patch), but I've gone through the cinematic Death fight three times now and I still don't have the bestiary entry. Oddly, both Death characters are still un-lockable.

Does anyone else know if there's some other criteria?

There isnt. You just have to wait if not updated on console.

After doing that four times now, I do wish you could skip some of it, as cool as it is.

I unlocked the character and that didn't update the bestiary. Neither did transforming into Megalo nor purchasing and playing Megalo. Just to try everything. So it would appear it is simply broken for the time being.
