As usual with DLC weapons, the related DLC stage, or stages in the case of the most recent DLC, ALWAYS have all required items on-map for all dlc weapon evolutions.

Yes, it is a nice aspect of the game design, nice QoL. It still helps to know the evolutions though. I don't usually pick up everything on the map.

Does the armor have to be leveled up all the way? I had it but my sword never evolved.

Yes, all DLC weapons require the items that go with them to be fully levelled.
