Evolution unlock achievement completed, reward not unlocked

I have a weird situation. I completed one of the new evolution unlocks in the Castlevania DLC (hand grenade to be specific) and at the end of the run, I got the notification about the unlock. But on the Unlocks page, the achievement still shows locked (i.e. it doesn't have a red X). I evolved it again in a second run but didn't get the achievement notification the second time. Both the weapon and evolution show up in the Collection.

I've closed out the game, restarted, etc. Still hasn't resolved itself. Any tips?

I've had a similar issue with Trident's damage counter.
The wording may suck too, I was dumb and left right after it spontaneously evolved (I banished it at rank 8, picked up something that somehow gave me +1 Trident level to evolve it).
700k was a LOT more than 40k but alas, I'll simply have to go and redo it before I round stuff out.
Maybe you'll have similar luck just trying again? You can always do an easier map for these achievements in general.
