did the music in the first stage get changed?

I don't know how you could select music tracks in the game.

Once you unlock the Magic Banger Relic, the option becomes available to you.

Are you playing as one of the DLC characters or one of the coffin characters? Because five of the DLC characters and all of the others mentioned get their own unique music that overrides the stage music.

I've been using Gennaro for a bit but tried using other characters like Imelda or poe and the music still different.
I can't remember is any of those characters i unlocked from a coffin.

Coffin characters are: Pugnala, Giovanna, Poppea, Concetta, Zi'Assunta, and technically Miang (the first DLC character).

All the main stages have secondary tracks. For whatever reason, Mad Forest defaults to playing that instead of the original.
