What exactly do I do after getting the yellow sign?

It says there's a relic in Cappella Magna, but i have no idea how to get it. The weird hand things will say what direction those rings are, but they aren't visible on the map? how do I find them? do I just guess? The hand shows up and snaps its fingers a few more times during the run but I have no clue what it's doing.

Yellow Sign unlocks the 4 bonus stage items. There is one in each cardinal direction.

For the items to show up on the map, you need the The Milky Way Map[vampire-survivors.fandom.com]

The Relic in Cappella Magna you get once you beat the main boss of the area.

I already have the milky way map, but they don't show up on it. Will they ever be visible or do i have to find it without any indication other than the direction?

The reason you aren't seeing the Arrows and Ring Yellow Items on Capella Magna, for now, is because they are disabled until you encounter and Kill Ender Capella Magna's special Boss. Once you kill him and acquired the Relic he's protecting you will be able to play Capella Magna's alternate form that allows the Arrows and Rings to Spawn.

In all other maps the Yellow Sign Items will spawn normally, Capella Magna is the only exception until you beat Ender.
