So about the lunar eclipse thing...

I was able to get the special stage, but didn't make it far enough to get the item.
For the "finish the lunar eclipse event starting from Moonglow"... does it matter that I failed the first time? Do I have to get the first part to trigger in Moonglow and get the item in that run? Or do can I just select the stage itself now that it's unlocked and get it there for the achievement?

You should be able to select the hidden stage from the level select assuming you didn't quit out of the game after losing and didn't pick a hidden character (IE: Toastie, Smith, Mask of Red Death, Leda, etc) after starting a new game if you die on it.

If it isn't available and you've chosen a non hidden character then you have to do the Moonglow part again, but this should only happen if you pick a hidden character or quit out of the game between failing the lunar eclipse event and trying again.

Yes, no matter how many attempts you take, you still get credit for completing it. It shouldn't take many attempts, unless you're using a particularly low starting stat character, as it is relatively easy to do once you actually get there.

Holy Forbidden only appears for non-secret characters.
So if your plan was "Well NOW I can cheese this with Leda/Death!", no, you cannot.
Antonio's +1 Armor and the reliability of Whip are probably the best one to use.
