Howether they do Teleport, not spawn.
I don't know if they would agro you from the whole map (that would require to never touch the item but stay in place as an AFK build) , but if you go near the item, then start going toward the original place of the ring/metaglio (says, far-left or far-right for the library) , eventually you will meet the guardian that are walking toward you.
So I would argue that Mad Grove attract the item and do activate the guardian, but they don't attract the guardian themselve. So if you go near the item maybe at one point the guardian will reach out to you.
Touching the item (where ever you discard or take them) would insta-Tp the guardian to your position tho'.
If you use the arcana that teleports stage items, they will teleport over when you interact with the item, but aren't near it.
And yes, you can kill them, they just have a lot of health.

With high Curse they can quickly become an inescapable death sentence since their damage is also high, they're knockback immune, and can only be frozen by the pickup item.
Mad Groove won't pull them until you pick up their associated item. This is good since Yellow Sign permanently unlocks those items, but places them out of your way. If they aggro'd from Mad Groove, the arcana would lose its value for some runs.
