But with Lauriel upgrade and Time Lancet (the thing that freezes enemies) I managed to get to min 32 back when this achievement didn't exist.
Not sure if it still works, but that's my only lead so far.
It's max HP is 655,350, MULTIPLIED by your character's level when it spawns.
It does 65,535 base damage. No amount of armor will save you from that. The only way to get around that is just not taking damage in the first place.
It has 1200 movement speed. You can't run from it.
High levels of knockback are actually bad against it, because it has negative resistance, meaning it will instead be pulled towards the player when hit.
It CAN be frozen, meaning the Clock Lancet (Weapon) and Orologion (Clock pickup from breakables) are essential to being able to kill it.
Another reaper will spawn every minute after the kill screen starts (31:00, 32:00, etc.). These will have different stats to the first reaper if the player leveled up after the first one spawned.
There are a few methods you can use, but the general rule is that you need to stay as low-level as possible until after it spawns, and as Panther said, Laurel and Clock Lancet are really good to get (If you don't have most of your re-rolls unlocked, it might be good to get those before trying this challenge, just so you can try and get what you need somewhat more reliably.
Also, don't try this with Curse leveled even a little bit. Save yourself from that pain, don't make the same mistake I did.
Do a lower level run, get Clock Lancet, and DO NOT get autotarget weapons like magic wand and it was actually pretty easy, I almost lasted to 32 minutes. With full duration upgrades, a Clock Lancet freeze lasts about 10 seconds, so you only need a few hits to last the 1 minute, and with full cooldown reduction its firing about every 1/2 second. As long as you don't have massive screen spam jerking him to you in less than a second its not hard to keep him off you, and if you take the Tiragisu and extra revives for 5x total revives, you can basically guarantee 31 minutes because with the couple seconds of mercy invincibility you're basically guaranteed a freeze for every revive plus the time it takes to kill you.
One is the immortal porta build, where you use portas reduced cooldown reduction for the first few levels to make laurel invincibility frames permanent. This method is near impossible so I'm not going to detail it here.
Another method is to unlock the secret character Toastie. He starts with one hp, but becomes invincible at level 200 (he gains 65520 armor, versus the reapers 65535 damage you cannot be killed by them). This is difficult but possible, focus on song of mana early.
Finally, the easiest method is to use suo clerici. Because clerici gains increased weapon size for the first few levels, he can stand in a particular spot in the inlaid library and become invincible. See picture below. Once invincible you need to avoid getting experience until after the reaper spawns. Then make a mad dash for the exp, level, and use the geometry to get major damage off on the reaper. We use lancet and laurel here too to stay alive until he dies.

She is really good with those 2 weapons.
Actually, Toastie gains exactly enough armor at level 200, and gets boosted to 10000 hit points at level 100. Toastie is basically designed to help you kill death.
That is if you`re an idiot like me and have 5 hours to spare just looking at the screen waiting for one of the 200 reapers to die that are flooding your screen and slowing the game down.
I'm pretty sure the strat is to survive on level 2, kill everything/survive somehow, and powerlevel after 30 minutes before death arrives and chops you in half.
