So saving up the pre-10 minute chests you get from the early bosses wont give you evolved items if you open them after the 10 minute mark.
Also in the Dairy Plant, all chests are capable of evolving weapons. It is an exception to the ten minute rule present in the other areas.
As for why it's not described in the game, this isn't out of the ordinary. Vandalier is the odd one out for not requiring a passive item to get, but I guess the trade-off is that you have to level it instead of it starting at full power like Phieraggi or the other evolved weapons.
There's no explanation for any of the combinations, probably to encourage natural discovery. Personally I'd rather games to allow players to figure things out through natural progression / trial and error rather than just telling you everything
I already have the combos memorized, but I thought the last patch made it so the game indicates when a new item pairs with something you have?
Whenever you get an evolved weapon, the game tells you which passive it required. So your first evolved weapon would be a surprise if you weren't aware, but after that, you'd know that you probably require something. After that it's a matter of either spoiling yourself or simple trial and error.
Well, that's just the nature of this game. It has a heavy emphasis on discovery

You're of course welcome to wiki the answers, it's part of the surprise of the game to find new combinations you weren't expecting.
Which is also why you should just never put a 'never' on any item in the game. While there are still some that don't upgrade, there aren't many.
Because you're supposed to experiment and figure it out.
If you get gold bags from a chest, then you should immediately know that you don't have the right combo. So retry and get different items.
It basically tells you "All the items you currently have are not the right one. So restart and pick all new items."
No items overlap, all items are used in one single evolution only.
For both items that require two weapons the hint is the weapons are basically identical but changed color. Once you have both guns maxed and it doesn't evolve you should know it must need another item in the combo, and that item is not one you currently have, so dive into your item collection and set up a new build that uses all different items until it triggers.
Once it triggers the evolution you now know that combo forever because the evolution tool tip states what the combo item is in collection.
