Yes, items have a max level, you can see it when you level up and can see the list of item you have in the top left corner.
To be more precise, it is a heal, not a health increase.
My first ever run earlier on when I did really well, I had no idea the items needed other items to evolve. So, here I was sitting there with tons of maxed items and mad that none would evolve. Kept getting endless streams of chicken or gold from chests and level ups. It felt very frustrating but only because I didn't know how the evolve mechanic worked.
I believe there should be some kind of tip in the game (if there is, correct me) that at least says "Some items can evolve if paired with another specific item." No details of what works, just a note that it is a thing so you're not in the middle of a great run and just thinking the game is broken or something like that.
In my last ALMOST successful run, dies at about 28 minutes, i got probably 25 money bags level up. I was very happy to get those since they are 25 gold each.
