You forgot the Meta Curse Power up in the Powerups menu that adds +10% Curse per upgrade level. So no, the Skull'o mania in game item isn't the only way to add curse onto your runs.
Curse increases enemy speed, health, quantity, and frequency.
You add curse to your runs in three ways.
1) Power Up from main menu. Max 50%
2) The character Lama gathers curse as the character levels. Starts with 10% and then gathers another 20% as levels.
3) The Skull o' Maniac is a support item you can pick when leveling up. Max 50%
The maximum curse is 130%.
The Power Up option is available for purchase with gold. You need to do at least one rank in this for 20 minutes on any level to unlock the character Lama.
You need to reach 30 minutes on any level with Lama to unlock Skull o'Maniac.
Once it's unlocked, and on a different run, you need to select Skull o'Maniac as a level up option to get it added to your collections.
A hard run is Lama + Skull of Maniac.
Harder would be Lama + Skull of Maniac + Hyper mode.
Harder would be Lama + Skull of Maniac + Hyper Mode + Green Acres
Almost impossible would be Lama + PICK Skull of Maniac first and level first + Hyper Mode + Green Acres.
I think the power up should either be priced at 1 gold, or not on the power ups at all. If it were 1 gold, you could pick it last in your PowerUp build order and the dev could add a small button that lets you just refund that one instead of everything.
It's not too harsh. It's purely asinine that game difficulty can be RNG scummed out of the player's hands at the 10m or 15m mark, thereby forcing a reset. It's completely stupid and shoddy implementation.
The obvious choice for the Curse upgrade as an extra, permanent, infinitely stacking option at level up. This lets the players pump as much difficulty as they want during a game while also giving a purpose to not only completely optimized and capped builds but also to continuing to level after finishing a build.
