Legendary Bosses - Im living legend
Beat them all, achievement just not popping, other achievements pop no issue...

Checked tactical guide and some are missing there, tho I did beat them for sure...

Example: DT boss is missing in tactical guide, but The Galactic Empire "folder" is 100%...so he cant even "appear" there in end...

Okay, for some reason I had to fight MagnaGuard boss twice... in case there were not 2 MagnaGuard bosses or some save discrepancy...

There is also a 'Reprogrammed' Magnaguard, that may have been it. Databank is useful to find which enemies you've missed. Unless you mean you had to fight the reprogrammed one twice, in which case it does sound like a bug. Glad you sorted it!
