For people who don't get Achievements
I had this problem since the beginning of this game - I wasn't getting any Achievements. After this patch I tried to do something with it on my own, since there wasn't any solution which would help me. So that's what I did and I'm hoping it'll help some of you:

1) Uninstall the game from Steam
2) Delete all files from Steam folder (if any files were left) - it's important especially when you had any mods
3) Uninstall EA Apps
4) Install the game again BUT ON DIFFERENT DRIVE (eg. firstly I kept this game on D, but now I have it on C)
5) Run the game and DO NOT CHANGE ANYTHING, DO NOT CHANGE ANY SETTINGS, LITERALLY DON'T TOUCH ANYTHING
6) Load save, go into the Meditation Point, take a rest, look into abilites, check everything you can check in there
7) The achievements should pop up for you

Welp, at least it worked for me and I hope it'll help at least some of you

Thank you for this. I stopped getting achievements after I installed the Outfit Manager mod. While it was very cool to play through the game with it on, I noticed the achievements stopped coming and thankfully was able to get all the missing ones using this strategy.

One point, though, I did NOT have to change to a different drive. Installing it in the same directory (after I deleted it, wiping out all mods and custom files) worked just fine.

I see, that's good it works anyway.

I was also playing on mods, but after some time I decided that I'd rather firstly get all my Achievements then go back to mods and that's what I did - I reinstalled the game, unlocked all Achievements and installed my mods again, so now I don't have to worry about anything xD

Anyway, once again I'm glad it helped
