Niche Issue: Can't execute Bedlam smasher

I've tried every weapon and stance in the game but no matter how many times I parry and get their life low and even see the glowing red circle no execution animation ever plays. I've even made sure no enemies remain but still no execution. Anyone else encounter this issue?
