Timed quest
Is there a timed quest?
I mean a quest that has to be started within a certain time.

Yes, there are some timed quests.

There are some that have timers (delivering stolen food before it spoils for example). Some of the main quests have timers as well but give you plenty of warning that the timer is counting down. In general, the vast majority of quests do not have timers and those that do are the exception

I understand, but do I have to start them to trigger the timer ?
If I don't accept it right away, will it remain on hold ?
Rule of thumb... if a quest involves somebody who is wounded and you are allowed to treat them, do that ASAP.

I haven't gotten through the whole game yet, I'm probably about halfway through the 2nd map. Every timed quest has had to be started by me and there would have been no issue if I had just been exploring around the map for a month beforehand.

I ask because I sometimes stay in one place for a very long time, sometimes just to explore.
Without starting a quest.
A few quests literally present you with mortally wounded people and if you played the first KCD's Johanka-questline, you should know they WILL die if you don't treat them.

I'm just afraid of missing things because I'm slow on quests.

Yeah I immediately finished that one... it's a close call when you find her.

Yes, you have to accept quests to start the timer.
