Trying to understand what this is. Is this item upgraded for some reason? Does it indicate it could be upgraded? Is it showing I have an arrowhead in my foot? These are traveller boots.

I believe it happens when you self repair your gear and it gets the corresponding buffs for doing so. Like footwear making sprinting take less stamina or whatever when you have that perk. Swords being 10% more damaging when self sharpened etc
Edited: I am entirely possible to be wrong here. I have seen no confirmation of this in game or elsewhere. It's just what I've deduced while playing and what makes sense to me in my mind lol

It indicates one of your perks has provided a bonus to that particular piece of kit.
Usually a craftsman perk.
Keen edge for example improves weapons you've sharpened yourself within a specificed range.
Shoes are a stamina bonus related to repairing them. I forget the perk name off the top of my head.
There are a whole list of others.

Up arrow afaik means "buffed", the only thing i know of that can cause that are your perks, e.g. bonus on repair or sharpening for swords
If you have the Seven League Boots perk under Craftsmanship, you will gain a stamina boost when sprinting with equipped boots that you've repaired yourself. The up arrow indicates you've repaired them and those boots will give you the buff.
