At least it was before patch, don't know after.
In the first map, the tavern in front of the blacksmith Radovan there is a guy, his name is something the mouth he tells stories, if you tell you one he'll tell you about brothers and a silver axe. The story gives enough clues that you can retrace the steps of the surviving brother, find the tomb of the other and get the pieces of the axe to reforge it.
It's just that it has a different name, not "protective axe"

Oh sorry I'm half asleep and I made a mistake, the protective axe is a quest in Kuttenberg, they're building a church and the woodworker who's been given the contract is anxious because he just took over for his father and he don't want to botch it. he wants to have an axe crafted with metal smelted from some relics that are not for sell.He'll place the axe in the church roof for protection.
The axe is a very basic one and not worth keeping
forged that one as well and no matter what skill you have, it's hardcoded to a certain level
You can sharpen it however like any other craft to improve it but its level is fixed
It's a quest in the first map, started by the romani witch woman, but maybe only if you had the Voivod give his amulet to his daughter.
The witch will ask you to forge a unique Rikonaris saber, which I was able to forge to level 4 (... and steal from her after the quest 😁).
I've also been able to forge the Adorned Axe to level 4, that's why I'm confused about other quest weapons being unable to be level 4. 😕
