I really down think it will be an issue. There were gay people in 1403 and we shouldn't get out of shape because it's in there are long as it sticks to the realism people like it for. Witcher 3 had it in the tutorial area and it was well written and applied to the time they were aiming for. Especially in the 1400s it would be a topic of taboo and mixed opinions. I think it's fine if it's in and the end of kcd it felt in place and not dramatic at all

Skill up is a fan - and whilst I dont agree with everything he says, and he is not a huge RPG guy he always has a solid take.
