Huh? You get a free roam time when you have to wait for Capon at Trosky before you headout to Nebokov. You get another one, when you return from the castle. After that you are locked until the second map, you need to do the find the gun quest, then you can free roam again. It gives you multiple instances.

Honestly I feel like the Kuttenberg map has been left purposefully empty so that further content can come and fill it up along with open more of the houses in the main city.
Nope, if you happen to ask Kybyenka about his gang, when you have arrived at the Devils Den, you (unknowingly) start a TIMED quest, and whatever freedom you had is gone again.
So much for freedom, stay hostage on the railroad train or GAME OVER..
