Experimental settings except GI set to low to increase cpu usage, dlss Quality

You sure the game is using all your cores? Did you disable some in bios or whatever? Now granted i'm early into the game. So not in a more intensive cpu area. However i'd suggest testing at 2k res and see what results you get. And my cpu vs yours is 14core/20threads vs 24cores/32threads. So highly doubt it's cpu bound unless it's Thermal throttling. And it would have to throttle a lot to be an issue i'd think. Note using all 20 of my cpu threads and my cpu temp is fairly low.
But no throttle or disabled cores
So I tested it here, please watch the video to see if you can reproduce it on your end.
I also took a screenshot similar to yours but my 13900k its being used differently per core.
BTW I couldn't reproduce the fps drops to 50 today, maybe it happens when you have a long time playing

Your cores are parking is part of your problem for stuttering. Run regedit and search for
Computer\HKEY_LOCAL_MACHINE\SYSTEM\CurrentControlSet\Control\Power\PowerSettings\54533251-82be-4824-96c1-47b60b740d00\0cc5b647-c1df-4637-891a-dec35c318583
Set Attributes to 0.
Open power plan options and check cpu settings. You should now have core parking options. Set P cores and E cores to 100% and that will disable core parking. Note Win11 here 24H2. I swear it used to be set both at 0%. Try 100% first as that works for me. Interesting that your cpu watts was 155 while mine was 95 with like settings. Parked cores you'd think help lower watts but i'd bet disabling core parking that there's no change in watts. I didn't notice any change after i disabled core parking. You may get in increase in cpu temps but looks like your cpu temp is well under control.
This should solve your stuttering issues. However the 78% gpu load when you still have plenty of cpu headroom left is very odd. I mean i've seen it in lots of other games. Typically Vulkan games like NMS for example. But we're D3D12. Unless it's an engine limitation. Interesting how we have very similar fps i mean off by only 2fps and my scene was a bit more crowded so mine should be slightly lower anyways. So very odd. Your cpu 5, 7 and 8 i don't like. Not a fan of 80%+ as this can lead to issues and my core 2 at 81% not a fan of either but at least it's only 1 core and not 3 of them at 80%+.
Did you hit Windows graphics. Under hdr set optimizations for windowed games to On. Next ensure KCD2 is listed there if not add it and set gpu preference=high performance and not let windows decide. and turn on optimizations for windowed games. I also have hags ON and Game Mode is On. Gamebar is disabled. Make sure nvapp overlay is turned off as it's buggy atm. Oddly enough Performance options/advanced tab set adjust for best performance of: Set this to background tasks. Yes it's arse backwards but oddly enough it fixes some games without hurting any.

Just tested with the core parking, I think it improves the % low just a bit nothing too crazy
I also installed the new nvidia drivers which seems to help a bit with the overall fps but not with the frametime spikes that much

I do like the cpu usage with core parking disabled. And see how watts are identical. They seriously need to remove core parking from Windows as it just causes issues and does not conserve energy. Maybe it's useful for laptops i dunno.
I lost some fps with new nvidia driver 572.70 but 0.1% lows were improved. I haven't rebooted puter yet after driver install ya i know shame on me so that may change. Try Vsync off via nvapp and set frame limit +3 over your refresh in the nvapp to help with screen tearing. You won't see the odd spikes doing this. Until nvidia fixes the weird timing issues this is what i do in most games i play. Typically only if i use framegen but for this game the spikes are pretty annoying so i do it here as well.
So below is with vsync off in nvapp no framecap. And i like to run a cpu stress test while playing a game. Just to see how well the game behaves. cpu-z was used for the stress test which cpu temp wise isn't that high compared to others. I did 2 laps around the town. Lower right is frameview benchmark as well. Didn't spike once or at least not that i observed. Checking the frameview csv file i did see a 0.1% low of 78fps.

I'm with 4090 and I use 4k with DLAA, Ultra settings (only textures and characters Experimental). I tried 4k DLSS Quality with all settings @ Experimental but I found out that there are places where you can get a bit of micro-stutter when rotating the camera. So I reverted back to native DLSS (DLAA) and most settings @ Ultra, which is perfectly smooth. One or more of the Experimental settings are causing this micro-stutter in some locations but I'm not sure which setting exactly, I didn't do more testing.
EDIT:
I did more testing, the problematic setting seems to be Vegetation Detail, everything else @ Experimental is fine. Try changing Vegetation Detail from Experimental to Ultra and see if it's better. BTW it's not CPU bound, with everything @ Experimental and 4k DLLS Quality GPU usage is usually 98-99%.
I also tried 4k DLAA with everything except Vegetation Detail @ Experimental but it's really heavy, I often drop to 50 FPS.

To fix the stuttering you need to reset your perks drinking the potion: Lethean Water, and in the 'Main Level' stat, DO NOT SELECT the 'Local Hero' perk, 'Local Hero' perk is bugged and causing stuttering in big places like Kuttenberg. Once you get rid of 'Local Hero', save your game and restart your game and the stuttering is gone.
