The Hydras Head achievement bugged?

Has anyone else noticed that that achievement doesnt count as complete even when the objective was fulfilled?

I did all the things I needed to on my second playthrough and yet even after the battle was over, I never got the achievement. Surely I cant be the only one thats had this issue
