They said in Dev update they're adding some barber options, as well as the ability to customize shields. But at the end of the day it's a first person game and you only see yourself on the paper doll. It's one of those things that isn't really needed.
it already will start with the first dlc, the one with the artist as it was said in warhorses official stream a few days ago that with that dlc you will be able to paint your own shields with symbols to add to it that get unlocked by quest progression.
my personal favorite helmet is the italian bascinet because it reminds me of gothic 2 paladins (obviously its not the same but close enough)
Outside of the aforementioned shield customization that is coming later, I am not sure what you are expecting in this regard since pretty much all the armor and equipment in KCD is based on some kind of real-world equivalent. This isn't going to be the kind of game series where you get fantasy style armor that you might find in something like a JRPG, Baldur's Gate, or Elder Scrolls.
