Need help with companions and difficulty modes!

I am missing a companion, I have Grimalkin, bakman, lawa, floating skull, ducky, vampire bat, simple cube, nun, lineth, dora. There suppose suppose to be 12, I only have 11. Also, how do you play in rearrange mode and hardcore?

For companions, you actually list ten names. koho should be in the underground part of the koho village, cactus should be in Meikan village(a scene with very little content, you need to jump down to get to the hidden part)
For different modes, start a new game from a different slot, choose light/normal first, then it goes to a new selection menu(only after you finish the game i think)

Head back to where you fought the harpy once you are later in the game! Its back in the first area where you got sprint
