Character cannot run - can only walk

I've come across a weird kind of issue during my game - sometimes I cannot get my character to run. I can walk, crouch, etc - but not run. Doesn't matter if I have a drawn weapon in my hands or not. The run / walk toggle does nothing.
I don't know what triggers it; when I launch the game it's OK - but something seems to trigger this issue - possibly going into combat and / or crouching, I haven't been able to narrow it down.
Saving, exiting and re-launching the game solves the problem - until next time, that is.
Rather annoying, though!

Anyone have any clues as to how to resolve this problem?

Did you press Z or LCtrl?

My L-Ctrl is bound to the 'Crouch' command.

Left Crtl is Crouch / Uncrouch

Z is Walk / Run
