I've known other developers who do this over the years too...
Console commands have disabled achievements since FO4, so it made sense this would carry over into Starfield...
Did they? I don't recall needing achievement enablers for either of those, nor for the original Skyrim...

Same things available in Skyrim/Fallout etc.
You probably dont remember achievements all together because they are utterly pointless and don't affect anything in game
Also you probably lost your old achievements anyways after buying the game again on a new platform / different edition
The first time that using a mod disables achievements got introduced in Fallout 4 game and got carried over to Starfield. The reason why a "M" flag (means "modded") is set on your game save is - likely - on request of Sony. The PlayStation trophies have a value on their platform. In order to allow a game to be modified on their platforms, they probably requested for this mechanism. Now, no achievements are being granted if your game save got a "M" flag.
As many has pointed out, it is easy to work around this issue by using another mod. It has its foundations here; https://github.com/Sumwunn/AchievementsModsEnabler/tree/master
You only "need" to unlock them once, once and for all. If you do care (I personally did complete them anyway for Starfield), you have options to do so in no time.
-keep a pure vanilla save and rush them to completion
-use an achievement enabler mod.
It happens that I encounter a profile that displays achievements for a specific game I've played as well. As I know some were hard or even reached under a specific circumstances (as they're hunting it...), I am intrigued. Perhaps it is a skilled player. So I checked the list to see what for achievements this person did as well ... to discover that all (or a big part) got done on the same minute. Yeah Yeah...

Yep. Initially FO4 was not coming to PS4. Sony and Bethesda had already closed negotiations. They reopened them anyway, with inclusion of this M thing for saves and excluding all non-vanilla assets from PS mods.

Only some authorised Creations... Otherwise most Creations also disabled achievements...
