Nope, he's not joking, you cannot save everyone in your first playthrough. You can, however, make sure to save whoever is most important to you by making sure that wherever they are is where you go when that choice comes up.

The only way to get Sarah killed for sure is to romance her and then not chose her during the event.

Funny thing though, you can go to Andreja literally right after Sarah dies and like "i love you". And she's like "i love you too". Next dialogue "let's get married, here is a skull of my goat, my most valued possession".

In the first playthrough I went to the Eye with Sarah and Sam died in the Loge. I've now gone through 4x NG Plus and Sarah is still alive.
Plus, marriage is forgotten once you start the NG Plus. Then Sarah doesn't know you anymore.

Somebody dies in a main quest. The choice is between the 2 you have the highest affinity with. You can save-scum it if you chose to save the wrong one.

There is a "dear friends" path other than marriage. I have no idea how that works out "points" wise though for affinity.

You get a big affinity boost for doing the "friendship" quest, and the companion moves up to "ally" or something like that. And it adds a new "check on Sarah's feelings regarding friendshp" dialogue option.

Poor Sarah. Spend her best years chasing a scientific career, then joined the marines, get caught in a conflict, was shot down and waited more month to be rescued. Got chosen by Arja to join Constellation, wasted more time.
