The only way to get Sarah killed for sure is to romance her and then not chose her during the event.
Funny thing though, you can go to Andreja literally right after Sarah dies and like "i love you". And she's like "i love you too". Next dialogue "let's get married, here is a skull of my goat, my most valued possession".
Plus, marriage is forgotten once you start the NG Plus. Then Sarah doesn't know you anymore.
