Sex is great in real life, and can be a natural part of a movie. Janky uncanny valley antics while you wait for the game to resume? Nah.
For real the sex scenes in the witcher were just awkward.
Anyway: I think they went for a lower rating tbh, and that's why there's no dismemberment etc. when there's blood, implied organ harvesting etc
This game is very family friendly. There is pretty much no real violence (most named npc´s can´t die) and no dismemberment, no real companions (they are more mule, than companion) and thus no need for romance.
This is essentially a Shooter Looter with Action Adventure mechanics and some building/crafting to bring along.. its not supposed to have any deep character interactions, if you notice every npc in this game, they are soulless blank slates, that just wanna give you x or y static quest and only reacts to this.
The game is rated M, because there is strong language, mild violence and some blood.. the thing that actually makes this a M +17 and not a +12 is because it has drugs included.
If you venture into a age rating place like ESRB, then you will notice that it even has suggestive themes, because you know.. it has to be mature, when a character can say something like this (then its sex)
