Emil Pagliarulo is the problem
Emil Pagliarulo is the design director and lead writer for Starfield, Fallout 76 and was senior director and writer for skyrim and Fallout 4. All games with bad (him as writer) to ♥♥♥♥ (him as lead writer) writing (story, dialoguie, etc). Him being design director also explains the archaic gameplay design of Starfield.

Now focusing on Starfield. sure its basically the same engine since 1997 but a designer with the experience of someone that has worked as long as him should have been able to think so far out of the ♥♥♥♥♥♥♥ that he would be able design ♥♥♥♥ to remedy these limitations. Like the design choice to show a black default loading screen when warping instead of a white one. A mod that turns that loading screeen white makes the loading screen so much more bearable yet it wasnt thought of.

No separate set of design documentation. No master design doc. It was all code-level comments. That's not how you do design documentation on a 200+ million dollar project, unless you're Emil, and unless you're incompetent as a lead designer.

I have never play Fallout 76, Skyrim and even Fallout 4. Plus Starfield is very buggy and has poor PC related dialogue.
ever since fallout 3 the series has just been on a constant decline (with the exception of new vegas of course)
And here was me thinking it was loading screens, or the Steam percentage score, or the Russian Moon base, or whatever. Make your mind up people.
I can't help but think that a new writing team seems essential.

To help stop them careering down that path from hero to zero.

I just saw a full hour video on this dude yesterday, it looks like he's the blame for HORRIBLE writing in all of the bethesda games. you can't defend this dude

After listening to those devs it sounds like a cult all enamored by their dear leader Todd. When someone is described as charismatic that should be throwing up red flags for anyone knowledge in psychology.

Charisma on its own isn't bad but when combined with the other personality traits it becomes problematic. You have an ambitious person who knocked on Bethesda's door demanding a job. You have a documented control freak often described as a seagull that flies in and ♥♥♥♥♥ on everyone's work. You have someone who will reject the first draft and accept the second.

You have the cult following devs who will say they got "bopped" but deserved it like women in domestic violence situations that say it was their fault. You have devs crunching and sleeping in theatres and cubicles. You have devs doing physical harm to themselves to please Dear leader.

You'll also notice why they all left when Bethesda got too big, their access to Dear leader was reduced. This is also the time Bethesda/ZeniMax became more unionised, first QA then all staff. No longer could crunch be a thing to appease Dear leader.

That culture that Emil was/is a part of as Todd's yes man lackey is only part of why they are now in decline. All cults eventually fail...
you know,i can almost see a Todd/Emil Design Discussion going down like Lin and Heller at the Intro.....

all the Q&As are EXACTLY Like Lin and Heller.
