In one breath you post casually about shooting another human being in the head, and watching the blood splatter.
The next breath you imply anyone wanting sex scenes will need mods from perverts.
Here is a newsflash for you, adult humans count for 73% of the 8 billion people on the planet, which means sex happens hundreds of billion times per year, each year, every year.
it's possibly the most natural act that happens between two consenting adults.
On the other hand, homicides globally is about 430,000 or about 0.001% of the amount of sex that happens per year.
You will happily run about playing a mass murdering hobo, and not think yourself odd for delighting in the many inventive ways you can kill in these games, but sex? hold on now! that's totally and utterly not acceptable.

You tend to find that games with violence are more common and far more popular than hentai games. There's a reason for this.
...whereas one person's idea of what is sexual content can make significant numbers of other people cringe, or skip/look away, or feel bad about buying the product.
This is the reason weird anime games aren't popular.
It's also a great reason to not want developers' explicit, graphical takes on what's sexy. Ponder whether you want your graphical sexual content foisted on you by game devs.
Leaving it out and letting you mod in what you want is probably optimal. It's a mercy that Bethesda prefer to do this imo.

We're talking about the social normalisation that views killing as entertainment, and sex and or nudity as a taboo.
Seems to me this is very questionable.
A sex scene where it's appropriate adds to the experience of the game (say for instance you pursed romance, married a companion and have a marriage night), Nudity where expected (Like a dancing club on a criminal world.) adds to the ability to suspend your disbelief.
No one here is asking for your companions to follow you about in thongs and sandals and bunny ears, and every time you enter your ship there is a bacchanal scene of debauchery in the sleeping quarters.
But if you're going to make an M rated game, then treat your customers like adults, and put in all the things adults get up to, not just the killing and criminality.
I can suspend disbelief without seeing pixel breasts and having to watch a cringeworthy scene of my avatar dry humping his partner every time he goes to bed. I'm an adult.
