I'll take 3 or 4 shots right out of the gate and eliminate 3 or 4 baddies. Once in a while I miss or they don't go down in one shot but head shots always eliminate them.
like fizzbin I also have marksmanship skill and rifles ranked up but when they were each only on Rank 1 i had the same accurate experience.
Or perhaps we are really just that unlucky.
I tend to take a body shot as my first shot, then head shot. It seems to do a lot better then sniping straight for the head.
You'll note that during charging, the T in your scope's reticle drops... ...this is IMHO intended to simulate the flatter trajectory the faster bullet will take. So when you fire without charging it up, it'll fire a less accurate shot...
Mobs generally have target arcs that obstruct certain limbs from being hit from certain angles. Headshots are meant to be blocked from behind on large humanoids.
Your first shot visually hits but is obstructed by the body so doesn't cause any damage. This likely causes the mob to turn towards you where headshots aren't obstructed.

...aim-down-sights-speed means the maximum transverse mobility of the weapon when aiming. The "follow a moving target"-speed, so to say. NOT the speed of target acquisition.
It zooms the scope if you have one and enhances the sight aiming and accuracy otherwise.
How that is tied to some sort of speed is a mystery to me so far, I need to play around with rifle mods in game to understand it more fully.
