At those times, she has a Novablast EM disruptor rifle with only the non-lethal mods, a single unit of heavy particle fuse, she doesn't have any grenades or mines, and none of her suit parts have an explosive, fire, or counter-attack effect.

I know she has Level 4 Concealment, but my head-canon is that adding a Tanith 1st-and-Only Cameleoline Cloak to a mutant whose skin that has preternatural, light-bending camouflage effects is nearing Raven Guard levels of stealth. With a side-order of heresy.
If I need maximum stealth, all companions get left behind.
It's a bit fiddly, but it works.

Double-checked and so she does. I know why I made the mistake; Andreja has the invisbility-while-crouched-and-not-moving buff that comes with Concealment 3. This activates even when she is not wearing armor pieces with the Chameleon effect, which does the same thing as Concealment 3.
When I first saw the effect, I said, "Hmmm..." then checked her stats. My cranial anxiety blob substituted "Concealment" for "Stealth" and I never thought about it any further before this thread.
