Report a translation problem
As people have pointed out you can do it by simply using the companion and they gain affinity.
You could also use the console to edit their affinity, rather than use a mod.
Since the CK is not out people should be trying to avoid using mods as much as possible.
On top of that I have been hearing rumors Bethesda did away with one of the file formats for mods, so that means Starfield is going to have a lower mod limit than Fallout 4.
Once you get to the lodge and you know the attack is coming you then have the choice of where to defend, either The Eye or The Lodge. That's the final decision on who dies.
As far as reading that the companion with the least affinity is who Vlad sends with you that's probably true although it's the one with the least affinity of the 4 so it will always be either Barret, Sam, Sarah, or Andreja, but like I said you choose where to defend which determines who dies.
Wahts the name of the mod?
Your highest affection will be option 1 so to avoid Sarah or Andreja dying don't get to friendly with them. This will also be true if you have a romantic connection with anyone, they will be option 1.
After this the choices are decided when you goto help with repairs on the eye ;
Option 2 will be the first person that you help with repairs on the eye.
If at this point you have no romantic interest or have not bothered to get friendly with anyone and therefore affection is low then the 2 people they you will need to decide on are the first to 2 people you help on the eye.

Romance Sarah
Help Sam first on the eye
You will have to decide if Sam or Sarah dies,
No Romance
Help Sam first
Help Andreja 2nd
These will be the people who you have to decide on and so on, this has been the case for me personally on all of my 10 run throughs but became apparent on about the 3rd time.
I intend to test this further when I can be bothered to start it up again (taking abit of a break tbh as I've become bored after NG+ 10) so please do feel free to correct me or share your findings, as I'm not 100% how accurate this is but I'm 85%+ atleast but 100% certain your highest affection will always be option #1.

I'm guessing it was because I was teamed up with Barrett pretty much the whole game so far.
Vlad will always send you out with whoever is your #2 affinity companion. For the OP - going around and murdering civs with Barrett is the opposite of what you want to do. Knowing this, you can choose to spend time on side quests/etc making "liked" or "loved" choices with someone you would rather accept as a lost companion ... ultimately someone has to go ... it'll make more sense soon.
You'll eventually arrive back at the lodge to begin High Price to Pay with the #2 companion. At this point whoever you choose to defend, between the Constellation members at the lodge or eye, that companion can be saved. GL
