I believe during this mission the game picks the top 2 constellation companions you've bonded with the most. One will be in the lodge, the other in the eye. Then the mission happens for reasons and you have to pick which place to save. The companion at the place you choose is saved and the companion in the place you don't choose is killed.
So in your case, it's probably Andreja and Sarah in play. If say Andreja is in the eye but you choose to stay at the lodge during the attack, she'll die. But if you go to the Eye you can save her, but that would kill off Sarah. So the one you don't help has to die.
For me Sarah was saved but Sam got oofed. Some say Barret is always a possible contender no matter what but I'm not sure if that's true, I didn't bond with him much and he lived in my playthrough.
I personally like the mechanic as it's bittersweet and think trying to engineer a disliked NPC dying takes away from that.
