Chameleon effect broken and how you fix it
Probably not the only one that has experienced this, if you use one or several items that give you the chameleon (invisible) effect when crouched, during combat, the effect will stay on your person even if you remove all the items that give this effect. The player stays invisible even when not crouched. This only happens during or after longer combat, typically not when just using it briefly.
Fix:
If this happens to you and you are not sure what to do, save the game and reload that save; if that does not work load a different save then jump back to your latest one, if it still persist close the game and reload, that has always worked so far.
Personally I will stop using such items until they or a mod fixes this because I don't want to risk not being able to remove it anymore and loose a bunch of progress.

I've had that happen to me and Andreja when doing stealth attacks. It won't harm the game because restarting from desktop always solves. It's the J%$*)(#% audio bugs that Bethesda needs to fix first, like either NPC dialogue being silent or random hissing noises that only go away if you restart from desktop.
I have this problem as well on game pass and it seems to happen when having multiple chameleon gear. I know there is probably meant to be some kind of benefit to having multiple chameleon gear, but this is the only work around I found to the big.

It's a well known bug and also the conflicts it can cause using Void Form. Until they fix it, the save the game, exit the game, restart the game and reload that save method, works.
Good tip, OP! Save/reload is the space game mantra to fix a variety of issues. NMS is the same way. Reminds me of this oldie but goodie:

I used to go out of my way to find Chameleon gear, but since leveling Void Form, I don't need it. I'll take the highest DR armor every time.

Maybe that's why I haven't encountered this issue?

Void Form will work if you cast it while you are walking just fine. But if you cast it while the Chameleon effect is active, when you move, it will cancel Void Form. But if you can cast it while moving, you can stop and move and it will not cancel it.

I like the max skill for Concealment because of the sneak attack bonuses. But the Chameleon effect is a package deal. :p

So you agree using Void Form without wearing Chameleon gear will avoid this bug? There's a quest where I have to cross a room filled with NPCs to reach a vent. I can do that with sneak + chameleon + creep VERY slowly without being detected. This takes 5 mins. Or I can cast Void Form, run across the room in 2secs and get into the vent without being detected.

I don't have Concealment yet. I haven't decided what I want to waste skill points on in that Perk tree in order to qualify for it :steamfacepalm:
