How do you throw a grenade?

I'm using an xbox one controller and now picked up a grenade in the abandoned research lab. But I can't figure out how to throw it. Can equip it and see it next to the gun on my screen bu cannot throw it... Do I need to gain a perk or skill to throw a grenade?

Yes, I even added it as a favourite and can then quick access and select it. Can see the grenade icon on bottom right of screen, next to gun, but it won't go into my hand or be thrown.

RB works fine for me, try a restart.

I think you need to hold the RB until you hear the clicking sound, then release the button to throw it.
