For those who have short attention-spans and don't want to bother reading a bit: the automatic doorway/ladder placement is sporadic and there is no real way to control it. The way Bethesda did it is moronic and further shows a lack of QoL improvements.
Definitely feels like interiors on ships were not tested much or thought about both from the crazy door and ladder situation and the lack of corridor and stair modules!
How I control door placement is I place my modules in ways that prevent any placement I don't want. It ends up spacing things out a bit, but it works - until it doesn't and my ship doesn't spawn an exterior or is rotated wrong on the dock base or ... bethesda stuff
