My companion died permanently, wtf
Andreja dead in a massive explosion. Not way to revive. Previously if they were injured they would go thru an injured animation & then get better. But she is just dead. Fast travel, nothing. Had to reload a previous save.

Yeah, that is pretty much how companions worked in Skyrim as well. Usually companions are very resilient to where they will go through the injured animation pretty much all the time, but if they keep taking damage for some time afterwards or a ton of damage all at once then they WILL die.

Yeah but i thought in Starfield that these Constellation members you get as companions were important NPC's & cannot die for story reasons?

They are protected, not essential.

If YOU do them damage while they are down, they die. So...

"essential" is a flag that an npc has if they can't be killed.

There are several levels of death protection of NPCs. ranging from no protection, to unkillable. The companions, at least the non story ones are merely 'protected' in that they will only ever fall down and crawl when they get lethal damage.

There are two levels of protection there too - one level they will only die if the player damages them when they are down, the other level they will die if damaged while down.
The game is teaching life lessons about following OSHA.

setessential is the console command to set an npc will not die.
