there isn't an ending for telling Lies all the time, but there's one for telling the truth only I think.
Endings be like: obay Geppeto or ♥♥♥ Geppeto. the third ending is the truth one and ♥♥♥ Geppeto too.
I still got the golden lie in my first playthrough but I did tell the truth once or twice, so not sure what count as a most do.
Yes, any activity that has red sparkles around you with text such as "your gears are reacting" increases your humanity.
1) Lying is the primary source of humanity
2) Listening to vinyl records for the first time (gold don't count)
-in NG+, I believe you need to "find" the vinyl again before you can get the humanity from listening
3) Lying at phone booths
-Going to the trinity rooms do not increase humanity
-Only one trinity door had a "different" reward in NG+. You will get +1 gear and equipment to continue to improve your character. The trinity door at the King of Puppets gives you a core that improves fire resist +1
4) Misc. actions can increase humanity
- Praying at the statue right before the cathedral stargazer
-Killing Alidoro after he has betrayed the hotel
