In my playthrough, I was always choosing to lie and P's hair changed two times over the course of the playthrough, as he was becoming more like humans than puppets. First his hair grew long and later his hair became gray (because of Sophia).
The change shape option alllows me to choose the hair I want: the initial short hair, long black hair or long gray hair.
I'm on NG+ & noticed I have long hair lol.
Yeah P only really changes appearance wise if you lie a bunch. Basically the more human P acts the more human he slowly becomes. So if you a run where you mainly tell the truth you'll stay a puppet and not get many change options, if any.
