Why do my base Pals keep getting incapacitated?
I set some pals to work in my base, I don't have assembly lines here so no injuries, and I go off to explore and catch some more. After like half an hour I come back and there's 2 - 3 incapacitated.

I don't have the motivation / observation station or whatever its called yet, and raids aren't happening. So why are my Pals getting incapacitated?

Is it the same reason why the NPCs at the "Small Settlement" keep dying and respawning when I fast travel away and back, which I assume is a bug?

They frequently get stuck on on objects/terrain and then starve or get injured.

They load in partially underground and fall through and hit the death barrier.

Or fall into holes, run off a cliff, or get on top of your house and cannot go down.

I also have the same problem, if I'm in the base they don't get incapacitated, but if I leave and stay outside the base for a few minutes they start taking damage from I don't know what and they die.
If anyone has any temporary solution

Multiple Reasons:

A) They get bugged on a specific furniture or mining or
B ) Some are too big and can't move properly and get stuck
C) Some pal has the the negative trait " Destructive or Unstable"
D) Some multitask pals are doing the same thing over and over again. They need to work different task.
E) Their sanity is low due to lack of beds, food, hot springs ( make bread for 4 san gain if you can as they give sanity back to your pals when they eat. you can give higher tier food but they are longer to make and takes more mats as well ) Otherwise berrries is fine.. just that bread is the 2nd best food for them )
D) Idling too much. Some pals hate idling. Make sure there is work.
F) Some pals needs break. It will ( on break ). Nothing you can do about that. They need to resd.
G) Bug - If Pals bugs out, just completely log out to desktop, then log back in.
I ) You exit the game while being raided and some Pals can't move bcz the "raid" is still on-going somehwre. You need to find the mobs ( wherever they are ) and kill them which will fix the issue. I have that problem before where I got raided.. I exited the game. Tnen when I relog in, all my pals couldn't move. That's bcz I had some mobs not too far that needed to be killed for some reason. The raid disappeared... but some mobs were still there no too far of the base. Once I killed them, all my Pals were back to normal.

J) A NEW BUG Unkown - If all of the above don't fix anything, you may need to butcher your pals, or let them go... or replace them.

Almost everything needs a decent amount of cleared space around it; some things I found out on my first Camp:

Placed my Palbox near the base of a cliff; Pals would get stuck on the cliffside and starve.

Placed beds on the 'patio' of my small cabin; Pals would get stuck on the roof.

Placed Wood Lot against a ruined wall; Pals would load in in the treetops and starve.

Placed Furnace next to Grinder; Pal couldn't get to the Furnace - had to move the Furnace a full Furnace diameter away.

Placed Pen near Cliff Edge; Pals would fall to the base of the cliff and starve.

Placed Chests in a single area; Pals spend all day walking back and forth - Chests are cheap, and you can craft from them, so put small Chests right next to Pens, Wood/Stone Lots, etc. Don't bother putting one by Fields; Pals will always put any Field output in their own food bowl (which means you need to keep an eye on them if you want some for cooking).

TL;DR: Keep things spaced out if you want happy, functional Pals.
