Pals randomly getting incapacitated?
This usually happens when i try to fast travel to base from far away. No campfire is present so i don't know how they die by themselves?

Some like to drown themselves if your base border extends over water by a significant amount.

There are absolutely no water nearby.

If it helps, base is near copper nodes with cathedral in red tree forest.

Falling through the world happens a lot, I had it happen to myself once and immediately turned off my hardcore server settings where I'd drop everything if I died.

Sometime AI goes on loop or some end up in places they cant get back down and die from hunger

i think its bc they get stuck and cant eat but thats the only thing i can think of

Games spawning them anywhere within the base circle, when they spawn(and this happens in most games to avoid clipping issues) they are dropped from mid air so they can fall down onto whatever game objects exist.
Problem is the game is spawning the pets before the game objects so only the actual terrain layer stops them, which means as things like rocks and stuff spawn they can get stuck inside. They can also simply be spawned into locations with no path for them, like up on a hill or in a ditch/water.
The navmesh for working pals(thing that tells the AI where they can and can't go) is limited to the base circle also so even if there's a ramp down/up a few meters outside the circle they still won't use it.

Then when they're stuck they just starve till stressed and then stress causes illnesses.
Best bet is to keep and eye out when travelling to base each time, if one is hungry pop him in and out of the pal box so he can respawn in a good spot so he can eat.

Also best way to prevent it is to find the flattest area to build your base with little to no rocks/water/cliffs etc.

This happens very fast though, like i was carrying stuff between bases and teleporting for every 30 seconds. They can't starve in that short amount of time, right?

Thanks for explaining the mechanics! From what i understand that my copper mining facility will have many victims until it's fixed :(

If you plan to stay in the base for a while, just quickly right click all pals out of the base and then right click them back in.

Takes 5 second, usually fix all the problems

This can happen for 2 reasons which I noticed:

1. When you port in / out of the base and your PC is somewhat..."lacking" to a point it takes longer for the textures etc to load, they can fall through the map and die.

2. Hunger / injuries if left alone they can die after a while though that happened to me only 1 time in hundred of hours of gameplay.

The first reason only happened / happens when I join my friend who is hosting, in my single player game it never happened to me at all.
If you have that forestry or how it's called, pals can glitch up on the trees without being able to get down and starve to death. Pals can also glitch through structures or just straight up glitch through the ground (I think) and get stuck under the map and starve to death. If you see that some pal is missing, go into the box, put them in, and put them back in their box so they respawn if they're not already knocked out. Usually I'm never away long enough for them to starve, so you should be fine.

Yep i already do that to "refresh" their ai, but sometimes they are already dead when i load.

When you travel too far away, the game will unload the area your Pals are at. They'll still be there using workbenches, but natural resource nodes will despawn. When you teleport back the Pals can clip into the world, or clip into structures and when the game tries to resolve this it can fling your pals into the air causing them to "Disappear" then suddenly become unconscious when they slam into the ground.

I've noticed pals flying towards the heavens while wandering. They eventually drop and die instantly. Maybe related.
