How to summon Malugaz, the Corrupt?

Just started a new run (new world with old char). So far I killed all three (old) bosses and figured out thtat a boss was swapped. However Malugaz is not in his room, what do I have to do to summon him?

You have to find 3 crystal shards, combine them in to a skull and place that in his room to summon him.

Once the room is located, Malugaz can be summoned by placing a Skull of the Corrupted Shaman on the rune in the center of it.
