The vertices of a square
I've run through and dug up many, many Forgotten Ruins and I've never seen a 'Temple of the Chipped Blade' there. Nor have I ever found a Chipped Blade outside of Azeos' Wilderness. Not saying they couldn't be there, I genuinely don't know for sure.
The places I've come across the temple seem to be roughly on the same radius as Azeos' spawn point from the Core, maybe a touch closer or further out but not too far away from that radius. And out at around the 750-800 tile radius - same radius as the main Mold Dungeon.
As I understand it though, they can spawn all over Azeos' Wilderness with multiples available in each world. If you start at the closest radius 1st then run inwards about a 100 tiles from Ivy's spawn then you should find one with the least amount of excessive mining.
Perhaps a few other folks would like to expand on this with their findings to help narrow the search down (or not).

haha I've been doing that area and I've pretty much got everything from the start to the slime checked
might have been too aggressive in my digs and missed it

DY5L3X14 was correct, in that the temple spawned in basically the same radius as the Azeos boss area. The room wasn't lit up like a lot of the temples and POI's are so I almost ignored it.

I think mine was about halfway between azeos and the start of the area
