Non-slut ending: happy end, wedding at Palace, NG+
Slut ending: happy end, wedding at Prison, cuckolding Emperor during wedding, free game until you get bored and decide to start NG+
Bad End: defeted at floor 5 or final battle. No real scene, only a bit of text
For non-Prisoner difficulty you can cancel Bad End, otherwise game over

There's technically only 3 endings in the game.
Bad Ending, Empress Ending, and Happy Ending.

There are however, special titles associated with certain requirements within another Ending. Since one of the hidden achievements is associated with it:
Keep first kiss and both V-cards while also getting the Empress Ending. for just one of the special restricted-Ending-specific titles.
There's also another achievement-specific one, which is: Obtained over 400 S. Levels and beat the game (Happy Ending, obviously).

Apart from those two, there's one more ending-specific restricted title, but it's not an achievement: Keep your V's v-card in-tact and reach the Happy Ending.
