To use it fly to the planet you want to use it on and then on planet screen the button is there.
You get a warning before you fire.
I had to try it out (as one does) before completing the game.
I felt kinda sad and guilty afterwards.
There is an achievement for using 5 Exterminatus in campaign. I wonder if the Grand Master will go bananas, praise my actions or just recycle dialogue.
But if you are wondering, no. It does not impact the game. Albeit, I didn't receive freebie from the reports.
The best way to use it is on planets that are far from your usual routes, that are at stage 4-5.
tbh. i think most of the cutscenes and inengine stuff is pretty exceptional for a 40K game (or Warhammer in general). it's not as flashy as Space Marine, but got more (better) athmosphere imho. .
and yeah: only use it on planets outside of your protected routes (Prognos, etc.).
What makes this one so notable (besides the look of horror and despair on Ectar's face as he forces himself to watch the carnage) is that unlike in every other 40K game featuring Exterminatus, this time, you as the player are the one who presses the button. All that death and destruction is your doing. Quite sobering, really.
Also, after Exterminatus, the planet backdrop changes, and you can see that it is still glowing like a burning coal against the backdrop of space. I'm not sure if the glow fades if you leave the destroyed planet and revisit it later, though.
Exterminatus should be an option to resolve the Morbus gate activations, though, as those missions can be quite a slog and extremely dangerous to your Knights if you choose to run them.
All it does is reset the bloom. The planet can still be corrupted and missions spawn.
