How do you play acts 2-4 in coop?
I can only select the act 1 missions.

Sanctum is the last level I can pick and we completed that

Tbh I am not sure what is classed as act 2 onwards, I just didn't get any achievements?

Oh, you had me worried, you got the whole 14 missions.

Regarding the achievements, some of those only work in Solo (the act of unlocking is done with Cyphers, which is a Story Solo thing).

That makes sense, weirdly I got the act 1 one through coop.
Do you have a list of levels and acts anywhere.
