Are the worms unavoidable damage?
I've tried some testing and haven't managed to figure out a way to avoid taking damage from the little caterpillar worms that jump onto you. Dodging does not appear to activate when they leap at you. Blocking will work only sometimes, if they jump at the center of your body; if they leap higher towards your head, they latch on and bypass the block. Trying to swing and kill them midair is unreliable.

This seems like a design issue but I need more information. It looks like the only way to avoid damage is to see them soon enough and shoot them quickly enough, which is not a satisfactory solution (half the time these things are hidden from your screen).

My question is, how does the game want and expect you to deal with them?

Have you tried shooting them maybe?
They are mostly pretty far away from you.

Sure some get a grab but most of them have audio cues so you can keep your eyes to the ground.

Like I said, shooting them and/or using the GRP on them is an unreliable or unsatisfactory way of stopping them from hurting you; 1) they need to be on screen and visible, and not hidden around a corner, 2) your shot needs to hit them and even the pistol has crazy spread, 3) you need to have enough battery/ammo (which you probably will, but it's still using a resource). Edit: I forgot, 4) they're small and they move quickly so it's easy to waste a shot either by missing with spread or wasting a shotgun shell on something so weak.

This is also a solution that definitely doesn't work well when there are several of them at one time. At this current moment, I literally do not believe you can do the "worm room" (area just past Elias with the large pipes and tight spaces) without taking damage.

yeah. you shoot in their general direction when they're squirming around, they should die. can kill several with one bullet. you can even shoot them when they're flying through the air at you. the aim is very forgiving.

The baton's offensive block mod kills them.
