I quit the game and came back and all was working again. Bug?
Ok, apparently the toilets need to be on your top floor. Since they can have a staircase going up to them, but not one going up higher from there.
I also have toilets on third floor where 2nd floor guests go, but nobody goes up to third floor to sit on the luxury benches I placed there.
btw "Starry night" (the bonus painting) - it does not belong to medieval times either.
OK, I just did some research and apparently, so did the developer (awesome job, Dev!) so it does appear that there would have been public and private toilets in those days, just not the way we would think of them today.
