Is anyone else having issues with quests? I've had 3 people give me dialogue that should have prompted a quest to appear and nothing did. I was excited because I finally got Darius up enough that the first date dialogue was initiated, he told me to meet him in the carnival at 5, but the quest never appeared and when I talked to him in the carnival area it was just normal dialogue. Hope if they patch this it gives me the quests I missed back.

I finished 7 out of 9 altars before the patch, finished an 8th one after the patch and went to dynus. now I can only fight him to confront and my previous altar confront is gone! I do not get that option anymore and cannot continue even if I have the altars complete
(fighting him to continue is NOT an option for me)

I already mentioned this in another post but just so you know: you can use the ingame command console to force-start these type of quests if they bugged out.
In my case I had this issue with Vaan's first date and was able to force-start the quest with the following command:
/startquest VaanHangout1Quest

So I saw another thread that talked about it and I'll copy the instructions below. I will say that didn't open the command box for me and just pressing ` did. The full post is under Sun Haven > Guides > p1xel8ted's Guides and it's called Activate Built-in Cheat Menu. It has a ton of command examples.

Load into the game first, and load your character.
Push ESC to bring up the menu
Push F1, Left Shift, W A S D all together
If done correctly, a password dialog will appear.
Enter "ImACheater" without quotes and click Confirm
You can now open a console with ` or press Enter

Example cheats:
/godmode True
/addmoney 1000000
